Parent/Guardian Survey
Dear Parent or Guardian,
Every five years, school districts are required to undergo an accreditation process to ensure we are providing a quality education to students. In preparation for the upcoming district accreditation review process, we are administering an AdvancED Parent survey. This information will be used by our school, the District, and the visiting accreditation team as a way to improve our school system.

We value your opinion. If you have difficulties completing the
survey, please contact the school’s front office at (904)259-2551. If needed, you can complete the
survey at the school, or we can provide a paper copy of the
survey for you to complete.
If you have children in multiple schools, you will receive a message from the principal of each school. Please complete a survey for each school site. Please note: If you completed a similar survey earlier in the year, we are respectfully requesting that you complete this survey. The parent survey is different from the survey administered in December.
